A campaign's readiness score is the proportion of its applicable requirements that currently show a pass.
The calculation
The formula Launch uses
Launch takes every requirement that applies to the campaign, excludes anything marked N/A, and divides the number of passing requirements by that remaining total. A warning counts against readiness but doesn't count as a hard fail; a fail always brings a campaign below full readiness.
Why N/A is excluded
A requirement that doesn't apply to a given campaign shouldn't be able to lower its score, since it was never something that campaign needed to satisfy. See Why N/A requirements are excluded from the denominator for more detail.
